When I last played I had soulvizier mod installed, it's built on the underlord, but adds monsters types and there's henchmen available they take up an artifact slot and are summoned by a skill provided by the artifact.
Also ups the difficulty by quite a bit and there's a whole new dungeon area and a new tier of artifact. It revamps the balance of some skills and disciplines and completely changes the rogue mastery into occult which is a more caster hybrid a bit like dream. Underlord felt pretty complete when I payed it.
